Paws n Music Membership Form

I wish to become a member / renew my membership of the Paws n Music Association and will make payment of £12, £6 per additional member for family subscriptions (applies to members of the family living at the same address), £14 overseas)

If joining after 1 July the rates are £6, £3 and £7. Membership runs from 1 January each year.
